In a medium saucepan over medium heat, whisk together milk, cream, vanilla, and salt. When bubbles begin to form around the edge of the pan, remove from heat and add chocolate chips. Let chocolate sit for 5 minutes undisturbed, then whisk until smooth. Serve with dippers of your choice.
