Mix water, sugar, vinegar, soy sauce, ketchup, pepper, salt, garlic powder, and hot sauce together in a bowl.

Arrange onion slices evenly over the bottom of a slow cooker. Place roast on onions and pour sauce over top.

Cover and cook until pork is slightly pink in the center, on Low for 6 to 8 hours or High for 3 to 4 hours. An instant-read thermometer inserted into the center should read at least 145 degrees F (63 degrees C).
